Tackle Football/Cheerleading Registration Fee $225.00

Please Note:                                                                                                                      Except in the case of a medical withdrawal ALL FEES ARE NON REFUNDABLE. Withdrawal for a medical reason requres a letter from a licensed physician.

  • Football Fee Covers use of all equipment and uniform (except practice shirt, practice pants, athletic supporter, girtle and cleats)
  • Cheerleader fee covers use of uniform. (except Sneekers & Skivvies)

  •  BIRTH CERTIFICATE WITH A SEAL AND A COPY REQUIRED
  • A COPY OF JUNE 2020 REPORT CARD ( ALL MARKING PERIODS)
  • THE MEDICAL RELEASE FORM NEEDS TO BE FILLED OUT BY YOUR PHYSICIAN
  • A REGISTRATION FORM
  • GET FORMS FROM LINK BELOW
