Please Note: Except in the case of a medical withdrawal ALL FEES ARE NON REFUNDABLE. Withdrawal for a medical reason requres a letter from a licensed physician.

- BIRTH CERTIFICATE WITH A SEAL AND A COPY REQUIRED
- A COPY OF JUNE 2020 REPORT CARD ( ALL MARKING PERIODS)
- THE MEDICAL RELEASE FORM NEEDS TO BE FILLED OUT BY YOUR PHYSICIAN
- A REGISTRATION FORM
