  Now Practice is at Seaside Park – on Iranistan Avenue (M – F  5:30 -7:30)

• A COPY OF JUNE 2020 REPORT CARD ( ALL MARKING PERIODS)
• THE MEDICAL RELEASE FORM NEEDS TO BE FILLED OUT BY YOUR PHYSICIAN
• A REGISTRATION FORM
